Safety Perception Survey Template

Performing routine perception surveys is an easy way to gauge how your workers feel about workplace safety. This template includes five questions and response trackers so you can get started creating a site-specific survey.

For the best results, you should ask workers questions specific to the programs and control measures you have in your facility. That way, you can better understand which areas of safety need improvement.
