PSM Mechanical Integrity Form

Under OSHA standard 1910.119, businesses must ensure the mechanical integrity of the following equipment:

  • Pressure vessels
  • Storage tanks
  • Piping systems
  • Relief and vent systems/devices
  • Emergency shutdown systems
  • Control systems (monitoring devices, sensors, alarms, interlocks)
  • Pumps

The mechanical integrity requirements include proper inspection and testing of equipment, training standards, written documentation (including inspection reports), and repair.

This form serves as a quick list of all these requirements that you can use to make sure your site complies. If you answer any of the questions on this form with a “no,” you’ll need to revise your mechanical integrity protocols until you can answer “yes.”
