Management of Change Request Form

OSHA’s process safety management standard regulates potentially dangerous changes to processes, people, and equipment. This falls under the management of change (MOC) regulation, which requires a very methodical and well-documented process for implementing modifications.

An important step in the MOC process is filing a “change request.” Essentially, it is a formal request for a change to any element of the MOC. This is critical to ensuring that you analyze the potential impact of any changes and that you document them as well.

Download this change request form for your team members to fill out and submit. Then, decide who will oversee approving, denying, or deferring change requests. Don’t forget to save all change requests for a project in the same place so you can easily find and reference them in the future.
