Forms & Templates > Hazards > PHA Checklist
A process hazard analysis is one of the most important types of industrial process reviews you can do. It’s a qualitative assessment of the process designed to help you find potential hazards and to address them accordingly. Conducting an analysis is easier with this PHA checklist from Frontline.
It includes more than 30 questions that you can ask to review your operating procedures. It’s also customizable, so you can expand on the provided questions or change them to better suit your site’s needs.
With this checklist, you will review minor changes to your process to ensure that they do not create uncontrolled hazards. If you answer any of the items on this list with “yes,” you need to check with your project leader(s) to see if you should do a more in-depth process review.
