Hot Work Permit Form

Hot work activities include things like riveting, grinding, and welding that create flames and sparks. These job tasks pose a hazard to everyone working in a facility if the workspace isn’t prepared ahead of time.

Permits give workers permission to start these activities after they’ve secured the area. A hot work permit is required when the work area is not designated specifically for hot work activities.

Use this free hot work permit form to ensure all your workers are following OSHA regulations. Each worker must fill out the mandatory checklist before starting. They must also get a signature from the facility supervisor and sign themselves—signifying their understanding of the hot work requirements they must follow.
