Return to Work Form

When an employee comes back from a prolonged absence, they may still be in the recovery phase. That’s why it’s important to fill out a return to work form.

With this form, you can document:

  • How long the employee was away
  • Whether their injury or illness was work-related
  • What type of injury or illness they had
  • What type of follow-up care you will provide

Before the employee returns, collect these details to ensure the safest transition back to work. Document a new form for each employee return, even if the employee has had an extended absence in the past.
