Facility Profile Template

Having an up-to-date facility profile is a great practice for emergency preparedness. In the case of an emergency, your profile will provide relevant details for response activities. The purpose of this document is to give an overview of the facility and its operations.

Our free Word template includes sections for the following information, although you can customize it to add or remove information as desired:

  • Facility contact information
  • Property owner or managing company contact information
  • Description of the facility (layout, operations, workforce, etc.)

Periodically review your facility profile to ensure that the contact information is correct. You should also make changes when you modify the layout or primary operations activities that take place onsite.
