Confined Space Entry Log Form

Keeping track of everyone who enters and exits a confined space is essential for emergency preparedness. This simple entry log will help you do just that.

It is broken down into three parts:

  • Worksite details (location, attendant names, meter readings, etc.)
  • Atmospheric readings (LEL, CO, H2S, and CO2)
  • Entrant log (name, time in and out)

These key details allow you to quickly discover who (if anyone) is missing in the event of a collapse. And it also gets your team in the habit of communicating their whereabouts. This keeps everyone vigilant and proactive, improving the safety of the entire worksite.
