What is a Pre-Startup Safety Review?
A Pre-Startup Safety Review (PSSR) is the final step before a new or modified process is brought online. It confirms that construction matches the approved design, safety systems are in place, hazard analysis recommendations are resolved, and personnel are trained before hazardous chemicals or energy are introduced.
For facilities covered by OSHA Process Safety Management, the PSSR is required under 29 CFR 1910.119(i), and it’s tied to Management of Change (MOC), which is why teams run the review as the closing gate of their change process.

Where PSSR came from
The Pre-Startup Safety Review wasn’t a formal requirement until recently. Before the early 1990s, the US had no process safety framework, and terms like “process safety” and “process safety engineer” weren’t common use. That changed after two catastrophes.
In December 1984, a release of methyl isocyanate at a plant in Bhopal, India killed thousands and drew global attention to the risks of highly hazardous chemicals.
Example of the importance of PSSR
On October 23, 1989, a series of explosions at the Phillips 66 polyethylene plant in Pasadena, Texas killed 23 workers and injured hundreds more, becoming the catalyst for reform in the United States.
In response, the Clean Air Act Amendments of 1990 directed the Department of Labor to create process safety standards for facilities using highly hazardous chemicals. OSHA published the Process Safety Management standard, 29 CFR 1910.119, in February 1992. It is built around 14 interconnected elements, and the Pre-Startup Safety Review is element (i).
What are the PSSR legal requirements?
29 CFR 1910.119 is specific about when a PSSR is required. Under 1910.119(i)(1), an employer must perform a PSSR for new facilities and for modified facilities when the modification is significant enough to require a change in the process safety information.
Before any highly hazardous chemical is introduced, the review must confirm that construction and equipment match the design specifications, that safety, operating, maintenance, and emergency procedures are in place and adequate, that the recommendations from the process hazard analysis have been resolved or implemented (or, for a modification, that Management of Change requirements have been met), and that everyone involved in operating the process has completed their training.
The EPA’s Risk Management Program rule carries a parallel requirement at 40 CFR 68.77 for facilities it covers, and the Center for Chemical Process Safety (CCPS) treats the PSSR as a key part of Operational Readiness within its Risk-Based Process Safety framework.
The stakes for getting this wrong are not only physical. OSHA cites PSM violations on a per-instance basis, so a single audit can give separate citations. As of 2026, a serious violation carries a maximum penalty of 16,131 dollars, and willful or repeat violations reach 161,323 dollars each.
